To order a memorial plaque for your loved one, please CLICK HERE to download the current order form. Plaques are mounted on our memorial boards in the Main Sanctuary. Lights by each plaque are illuminated four times a year when Yizkor is recited and once on the Yahrzeit of the deceased.
